  • Click avail if you want to use the form to avail services

​

  • Make sure to fill out all the questions on the form

​

  • Wait for a thank you message, and to the instruction indicated on it.

  • Before going to barangay office, make sure you bring all the requirements that included on the instruction

​

remember: Every services will not be valid without dry seal

  • Choose a program

​

  • Click apply

​

  • Fill out all the information that being ask

​

  • Remember that program could be set in limited participants or could be not

​

  • After you fill out all the necessary information, wait for the message on your email, (you will received thank you message and stamp.

​

  • You need to download the stamp certificate and present it on the barangay together with your valid id

​
